Vasion Forms provides a variety of form and layout fields that let you design a form however you want. Although fields are versatile, using the appropriate field type to capture specific data makes analysis more accurate.

Field Name

Description

Columns

Group fields into columns. Refer to the Additional Field Details section.

Content

Provide nonfield information, such as the form title, instructions, or an image. Refer to the Additional Field Details section.

Data Grid

Add fields to a line-item grid.

Field Set Create a title for a section of the form or a group of fields.
HTML Element

Show a single HTML element. HTML elements must be properly closed because incorrectly configured HTML can cause issues when you try to move or edit other form and layout fields.

Panel

Wrap a group of fields with a title and styling. Panels help you organize your fields and create a more appealing form.

Table Add a table with columns and rows in which you can add fields.
Tabs

Group sets of fields together in separate tabs. Click or tab navigate to view other tabs.

Additional Field Details

Content

Use the Content component to provide details or important information about your form. The information you enter here is for display only and is not saved as form data. You can format text using the toolbar and insert images.

The Content component does not support the following:

  • Tables.
  • Embedded videos.

Columns

Use the Columns component to display fields side by side. Configure the number of columns you want, then drag and drop fields into each column. Place layout components first, then add your fields within them.

Conditional Display

Use the Conditional field configuration to show or hide fields based on a submitter's input. This reduces unnecessary fields and streamlines the form completion experience.

For example, configure a "Family Relation" text field to appear only when a submitter selects "Yes" to the question "Do you have a family history of heart disease?"

Signature

The Signature field places a signature widget on the form. When a user selects the widget, a modal appears with the following options for adding a signature:

  • Upload an image
  • Draw a signature
  • Type a signature

If you use a Signature field as part of a workflow Signature Approval Step, select an assignee in the Signature Properties panel while the signature field is selected. Use the Assignee Label dropdown to configure assignees. Refer to Add a Signature field:. For details on using signature fields in a workflow, refer to Build a Workflow.
